Why is a punctured liner more serious than it looks?
The liner is not decoration. It is the vapor barrier that keeps the insulated core dry, and an insulated panel is only as good as its dryness. A fork puncture the size of a hand opens a path for humid air and washdown water into the foam. The foam absorbs it, the insulation value drops in that area, and the panel starts getting heavier and colder on the outside skin.
The symptom operators notice is not the hole. It is a unit that runs longer to hold temperature, or a cold spot that shows as condensation on the outside skin in a specific area. By that point the wet zone has usually spread well beyond the original puncture, because water migrates through the core along the path of least resistance and does not stay where it entered.
This is why a small liner puncture is worth repairing the week it happens. A same week repair is a liner patch and a sealed seam. A repair deferred for a season becomes a core section replacement, and a repair deferred for two seasons is often a full panel. The cost difference between those three outcomes is larger than almost any other repair decision on a refrigerated unit.

How is a wet or damaged foam core panel actually repaired?
The first task is finding the boundary of the wet area, which is not visible from the inside. That is done by probing, by weight, and by looking for the temperature signature on the outer skin. The repair is cut back to dry core, because leaving saturated foam behind a new liner traps the problem permanently and the panel keeps failing thermally.
Once the wet section is removed, the cavity is dried, new core material is fitted and bonded, and the inner and outer skins are restored. Bonding matters more than fastening here. An insulated panel gets much of its stiffness from the bond between skin and core, so a section held on by rivets alone will oil can and eventually separate at the perimeter.
The finish work is where thermal repairs succeed or fail. Every seam that was opened gets resealed with the correct product, edges are capped, and penetrations are sealed rather than caulked over. A panel repair that looks clean but has one unsealed seam at the top edge will be back within a year with the same wet core symptom in a slightly different place.

What about doors, gaskets and thresholds?
Door gaskets are the single largest source of avoidable thermal loss on a refrigerated unit, and they fail predictably. Compression set flattens the seal over time, a distorted rear frame leaves a gap the seal cannot span, and hinge wear drops the door so the seal contacts unevenly. Frost accumulating in a line at the seal is the visible evidence and it is the point to act.
Correction runs in order. Rear frame square first, hinges and door alignment second, then gasket replacement, then the sweep and threshold. Replacing a gasket on a rear frame that has walked out of square from dock hits gives a few weeks of improvement and then the same frost line. Both diagonals of the rear opening get measured before any seal is ordered.
Thresholds wear through because every pallet jack crosses the same twelve inches. Once the threshold plate is worn or lifted, water gets under it into the floor structure and the rear sill, which is exactly where a refrigerated unit can least afford water. Threshold replacement with proper sealing at both ends is inexpensive and prevents a floor and sill repair that is not.

How do floors, drainage and interior fittings hold up?
Refrigerated floors are usually extruded aluminum duct sections that let air move under the load. Those channels crack at the joints under repeated pallet jack traffic, and once cracked they let water into the structure below. Section replacement is the standard repair, matched to the existing profile so airflow through the floor stays as designed.
Drainage is the maintenance item nobody logs. Drain hoses block with debris and product residue, meltwater sits on the floor instead of leaving, and the standing water finds every seam and threshold weakness the unit has. How do you know how much of the core is wet?

Water migrates through foam along the easiest path, so the saturated zone is almost never the same shape or size as the visible damage. The repair is cut back to dry core, since leaving wet foam behind a new liner keeps the thermal problem permanently.
My doors frost up at the seal. Do I just need new gaskets?
New gaskets alone rarely fix it.
